By checking the box below and submitting this volunteer form, I, the Volunteer, hereby acknowledge and agree that I voluntarily participate in Community and Veteran Events, hereinafter referred to as "C.A.V.E.", events and fundraisers. I understand that my participation may involve risk of personal injury. I assume these risks and take full responsibility for any personal injury, loss or damage that may occur as a result of my participation. I do hereby release and forever discharge C.A.V.E., its affiliates, officers, directors, agents, employees, and volunteers from any claim whatsoever which arises or may hereafter arise on account of any first aid, treatment, or service rendered in connection with my activities with C.A.V.E.
